• AIDS Memorial Name & Story Submission

    • Contribute a Name to the AIDS Memorial 
    • NOTE: The deadline to submit names for inclusion in the June 2027 AIDS Memorial Vigil is May 1, 2027. Names submitted after this date will be included in the June 2028 memorial.

      Partners, friends, and family members may submit names to the AIDS Memorial. We accept all submissions of names where the deceased had a connection to our communities, and specifically those whose connections were within the Province of Ontario. You may wish to ask other friends and family members if the deceased person made their wishes known regarding an engraving on the Memorial.

      The 519 receives engraving requests throughout the year, however, we only engrave new names once a year, in advance of the annual AIDS Candlelight Vigil in June.

      Adding a name to the AIDS memorial is free of charge. Costs are covered by donations to the AIDS memorial fund. If able, we ask that you make a donation to the AIDS memorial fund to help cover engraving costs.

    • Annual AIDS Vigil

    • Every year, we invite those who have added a name to the memorial to speak out their lost loved one's name at the Annual AIDS Vigil. This event takes place on the Tuesday of Pride Week, which will be Tuesday, June 23rd in 2026.

    • Would like you to attend the vigil? And would you like to share your loved one's name?*
    • The personal information on this form is collected under the authority of the City of Toronto Act, S.O. 2006, Chapter 11, Schedule A, s. 136 (b) & (c) . Personal information is used for the delivery of The 519’s programs and services. Specifically, it will be used for the purpose(s) of: The AIDS Memorial.     • We are able to accept stories and memories in many different formats. you may attach a word/pdf file, photo(s), or video below. you can submit multiple files.  Some options you can consider include: 

      - Writing a letter to your loved one.

      - sharing a special memory or moment of joy you had with your loved one. 

      - sharing why your loved one was so special to you. 

      - provide a brief biography about who they were. 

       

      If you would prefer to answer  some prompts to create a memory/story, please feel free to answer the questions below. We will then combine and edit your answers into a story.
